Antony managed to avoid a potential red card after his frustration escalated, resulting in a deliberate foul on the Belgian winger by kicking out at him.
Both Antony and Bruno Fernandes received yellow cards, and earlier in the game, Sofyan Amrabat had also been cautioned for a confrontation involving Phil Foden and Jack Grealish.
The outcome of the derby kept Manchester City within two points of the early Premier League leaders, Tottenham.
Meanwhile, Manchester United find themselves in eighth place, closer to the relegation zone than the top of the table, following their fifth defeat in 10 league games.
Following their victory in the Derby, Manchester City has a full week to prepare for the upcoming Premier League match against Bournemouth on November 4.
On the other hand, Manchester United will return to action sooner, facing Newcastle, a team that had previously eliminated City from the Carabao Cup in the fourth round on Wednesday.
